General Surgery ST3 Interview 2027: Dates, Timeline and What to Expect

20 October 2026 · 9 min read

Preparing for the MRCS can feel confusing because Part A and Part B assess different skills. Part A tests your applied knowledge through written questions, while Part B asks you to demonstrate knowledge and clinical skills in person. This MRCS Part A vs Part B guide explains what each exam involves, how your preparation should change, and the common pitfalls to avoid. You will also find a practical checklist to help you plan both stages.

Key takeaways

Part A tests applied knowledge through computer-based questions at a test centre; Part B assesses knowledge and clinical skills in person.
Your revision should change between exams, moving from question practice to spoken answers and timed clinical stations.
The seven-year window to complete the MRCS begins when you pass Part A.

Sittings	Three per year, usually January, April and September	Multiple UK and international sittings across the year

Part A and Part B are two stages of the same examination, but they test your knowledge in different ways. Part A is a written, computer-marked single best answer exam. You complete two papers on the same day, answering 300 questions over five hours. The papers cover applied basic sciences and principles of surgery. Your preparation should build confidence in answering questions and making decisions under time pressure.

Part B assesses your knowledge and skills through an objective structured clinical examination, or OSCE. You move through 17 examined stations, each with nine minutes for the task, alongside preparation and rest stations. RCSEd describes ten-minute station slots that include one minute of reading time. Follow your college’s candidate instructions for the full circuit and timings.

This difference should guide your preparation: Part A requires accurate decisions across a broad syllabus, while Part B requires you to explain and demonstrate what you know under observation.

What MRCS Part A Involves

Element	Details
Format	Two computer-based SBA papers on one day, five hours in total, at a test centre
Paper 1	Applied Basic Sciences — AM, 3 hours, 180 questions
Paper 2	Principles of Surgery in General — PM, 2 hours, 120 questions
Total	300 single best answer questions
Passing	Minimum competence in each paper plus the combined pass mark. Modified Angoff standard setting; the pass mark varies by sitting
Fee and sittings	£670 for January 2027 (RCS England); three sittings per year, usually January, April and September

One of the biggest challenges in MRCS Part A preparation is the range of material to cover. Aim for a solid understanding of applied basic sciences and principles of surgery, and use question practice to identify topics that need deeper study. With 300 questions across five hours, time management matters alongside knowledge and familiarity with the question style.

A good question bank can help you recognise common patterns, identify weak areas and become comfortable making decisions quickly. While practising, review why the correct answer is right and why the alternatives are wrong. Part A uses standard setting based on the modified Angoff method. The pass mark can vary between sittings to maintain a consistent standard, so there is no fixed pass percentage. Read the ICBSE explanation of Part A standard setting for more detail.

Common pitfalls include revising anatomy in isolation, overlooking Principles of Surgery in General, and leaving full-length mocks until the final days. Connect anatomy to clinical questions, cover both papers consistently, and leave enough time after each mock to address weaknesses.

The distinction between MRCS Part A and Part B is useful here: Part A rewards broad knowledge, accurate recall and efficient problem solving. Your revision should give you regular opportunities to practise all three.

What the MRCS Part B OSCE Involves

Element	Details
Format	17 examined stations with nine minutes for each task, plus preparation and rest stations; reading time is additional
Broad content	Anatomy and surgical pathology · applied surgical science and critical care · communication skills · history taking · clinical and procedural skills
Components	Applied Knowledge and Applied Skills — both must be passed
Station marking	Each station marked out of 20, with an overall examiner judgement (pass / borderline / fail)
Eligibility	Pass Part A first; complete the MRCS within seven years of passing Part A; normally four Part B attempts
Fee	£1,212 for the UK January–February 2027 sitting (RCS England)

In the MRCS Part B OSCE, you perform tasks while an examiner assesses you. With nine minutes for each examined station, your thinking and explanations need to be clear and focused. Tasks may include examining a patient, taking a focused history, explaining a procedure or demonstrating a clinical skill. Communication is central: structure your clinical reasoning, explain it clearly and involve the patient when appropriate.

The MRCS Part B OSCE also tests applied knowledge. You still need to recall and apply anatomy, surgical pathology, surgical science and critical care. Compared with Part A, you must explain or demonstrate your knowledge rather than select an answer from a list. Both the Applied Knowledge and Applied Skills components must be passed.

A common preparation pitfall is practising in silence. Reading sample answers can be useful, but you also need to speak your answers aloud and work through scenarios under timed conditions. Practise communication stations alongside clinical skills, and make sure you can explain anatomy in a viva-style setting.

How Preparation Changes Between the Two Exams

The main difference in MRCS Part A vs Part B preparation is what your practice involves. Part A requires you to apply a broad knowledge base to written questions. Part B requires you to demonstrate that knowledge clearly under observation.

For Part A, combine frequent question-bank practice with spaced repetition. This keeps revision active and helps you revisit topics you find difficult. Review explanations carefully so you understand each answer. As the exam approaches, complete timed blocks and full-length mock exams. Use the results to identify gaps and adjust your revision while there is still time.

After passing Part A, keep your knowledge active while converting written understanding into spoken explanations. Practise answers aloud, rehearse examinations with colleagues and build a simple framework for each type of station. For example, a Part A anatomy question might ask you to select a muscle’s nerve supply from several options. In Part B, you may need to describe its anatomy, explain its clinical relevance and answer follow-up questions. Recognising these different demands will help you plan your MRCS preparation more effectively.

Eligibility Fees and Timing

Rule	Details
Order	Part A must be passed before sitting Part B
Time limit	The MRCS must be completed within seven years of passing Part A
Attempts	Normally four attempts at Part B; an additional attempt requires approval under the intercollegiate policy
Listed fees	RCS England: Part A £670 (January 2027); UK Part B £1,212 (January–February 2027)
Sittings	Part A: usually January, April and September. Part B: multiple UK and international sittings throughout the year

Planning both exams together can make the MRCS pathway easier to manage. You must pass Part A before sitting Part B, so choose a Part B date that leaves enough time for focused preparation. Consider your clinical rota, study leave and access to practice partners before booking.

For the January 2027 Part A sitting, RCS England lists a fee of £670. Its listed UK Part B fee is £1,212 for the January–February 2027 sitting. Check the fee for your chosen college, location and sitting before booking, and allow for any travel or accommodation costs. You normally have four attempts at Part B and must complete the MRCS within seven years of passing Part A. An additional attempt may be available by application under the intercollegiate policy. Give each attempt enough preparation time and build a realistic timeline around the demands of the exam.

How to Plan Your MRCS Preparation

A clear plan makes MRCS preparation easier to manage. For Part A, build a consistent study schedule with regular question-bank practice and spaced repetition.
